By selecting this box, you, the Customer, are agreeing to a Short Term Service Agreement, beginning on the Service Start Date and ending on April 30, YEAR. Following April 30, YEAR, NFTC will continue providing Internet service on a month-to-month basis, unless the Customer contacts NFTC's Customer Service Department at: 1-888-638-3575 or support@nftctelecom.com to request a change. By agreeing to NFTC's Short Term Service Agreement, the Installation Fee of $300.00 will be waived entirely.
